New Customers
New customers must sign an Application for Service to begin receiving services through the MTMA. The application can be picked-up at the MTMA office, mailed or faxed to the new customer, or new customers can click
on the link below to print a copy of the application and either mail or fax it back to the MTMA offices.
As of October 26, 2009 all customers that sign up for service must also
submit a copy of their driver's license or passport with their application for
identification purposes.

application when signing up for service.
Residential Application
This application is for new customers that have
purchased an existing house or newly built
house without a fire protection system and are signing up for
service with us.
Residential Application for Newly Built
Houses with a Fire Protection System
This application is for new customers that have purchased
a house
built after January 1, 2011 with a fire protection system
and are signing up for
service with us. For more information about fire protection

Commercial Application
This application is for new businesses that have either purchased
property or are a tenant.
Rental Application
This application is for owners that have purchased residential property
that they plan on renting out to others. Please Note:
The property
will always remain in the owner's name, not the tenant's name.
